North Yorkshire telephone health coaching service is highly commended

A telephone-based health management service which aims to support patients at increased risk of unplanned hospital admissions in parts of North Yorkshire has been highly commended at a national awards ceremony.

The Health Service Journal Partnership Awards celebrate the most effective partnerships with the NHS and highlight the benefits working with the private and third sectors can bring to patients and NHS organisations.

Proactive Health Coaching‘, a service designed by health technology company Health Navigator in partnership with NHS Vale of York Clinical Commissioning Group (CCG) and York Teaching Hospital NHS Foundation Trust (YTHFT), was shortlisted among 10 other projects in the HealthTech Partnership category for supporting people with long-term conditions to improve their health.

The service, delivered by registered nurses and healthcare professionals through weekly telephone coaching calls, has celebrated a 36% reduction in A&E attendances for those patients supported by health coaching compared to patients that had not received the intervention. 84% of people reported an improved quality of life and having more confidence in managing their conditions.

Dr Andrew Lee, the CCG’s Executive Director of Primary Care and Population Health, said: “We are extremely pleased to have been ‘Highly Commended’ alongside Health Navigator and York Teaching Hospital NHS Foundation Trust. This recognition demonstrates how our partnership working has had positive health outcomes for patients.”

The HSJ Partnership Awards ceremony was held at Park Plaza, Westminster.
