Help make a difference in North Yorkshire

As a community connector, you can help young people have their voices heard.

You will be directly involved in making a difference for communities by letting us know about the experiences and issues young people have when accessing health and social care services.

Your involvement will help us to raise issues and concerns with those who have the power to make change happen.

How you can help us find out what matters

Community connectors will support local Healthwatch to tackle health inequalities. Health inequalities are the differences in the status of people’s health, but also the care that people receive and the opportunities that they have to lead healthy lives.

NHS England have identified five clinical areas that require improvement to reduce health inequalities in children and young people. This is called Core20PLUS5. Community connectors will listen to anyone’s experiences of health and social care services but there will be a focus on gathering feedback from people who have experience with the 5 key clinical areas, these are:

  • Asthma
  • Diabetes
  • Epilepsy
  • Oral health
  • Mental health

How can you gather views

Community connectors will be supported to engage with other young people in a number of ways, including:

  • Planned engagement activities with other community connectors and Healthwatch staff.
  • Talking to and gathering the views of the public and reporting back to Healthwatch.
  • Helping to conduct surveys and consultations in the local community.
  • Produce written case studies to highlight people’s experiences.
  • Promote the ‘Young people’s experience of health in coastal communities’ survey with other young people.

Where do we want to reach?

The Humber and North Yorkshire Health and Care Partnership (also known as an integrated care system) have identified coastal communities as an area for improvement.

The feedback gathered by our community connectors will be used to shape and improve care along the coast for other young people.

In addition to North Yorkshire, other Healthwatch are looking to hear the views of people in North East Lincolnshire and East Riding.

How you can get involved

We want to recruit young people aged 16 – 25 years to speak with other young people  around their experiences of accessing health and social care, any barriers they may have faced and any solutions they may recommend.
